The following is a list of health plans that we currently participate as In-Network.    Even if you are not a member of one of the following groups we will still be happy to submit a claim form on your behalf for your visit.  Please keep in mind that it is your responsibility  to determine if your insurance policy considers our practice as In-Network or Out-of-Network.  At your request, we will be happy to assist you in determining your coverage for both in-office visits and hospital visits at Brookwood Hospital.   Copays and deductibles are collected at the the time of the visit as required by your insurance company.   We recommend bringing your insurance card with you at your time of visit.
